Video abstract of Review paper entitled Management of primary dysmenorrhea in young women with frameless LNG-IUS published in the open access journal Open Access Journal of Contraception by D Wildemeersch, S Jandi, A Pett et al. Abstract The objective of this paper is to discuss the potential advantages of intrauterine treatment with a frameless levonorgestrel (LNG)-releasing intrauterine system (IUS) in young women presenting with primary dysmenorrhea associated with heavy menstrual bleeding. The paper is based on clinical reports of 21 cases of primary and secondary dysmenorrhea treated with the frameless LNG-IUS. Three typical examples of young women between 16 and 20 years of age, who presented with moderate-to-severe primary dysmenorrhea associated with heavy menstrual bleeding, are presented as examples. Following pelvic examination, including vaginal sonography, a frameless LNG-IUS, releasing 20 μg of LNG/day, was inserted. The three patients developed amenorrhea, or scanty menstrual bleeding, and absence of pain complaints within a few months. We conclude that continuous, intrauterine progestogen delivery could be a treatment of choice of this inconvenient condition. In addition, the good experiences with the frameless LNG-IUS in other studies suggests that the frameless design may be preferred over a framed LNG-IUS, as the absence of a frame, resulting in optimal tolerance, is particularly advantageous in these women. About 75% of the population is deficient in vitamin D. I believe vitamin D is the most important fat-soluble vitamin—especially for the immune system. Studies on vitamin D: 1. In the first study, the group took 1,000-2,000 IUs of vitamin D 1-2 times per week for 1 month and did not increase their vitamin D levels at all. 2. In the second study, the group to 1,000-2,000 IUs of vitamin D, and it took 4 months to elevate their vitamin D. 3. In the third study, it took 3 months to achieve the normal level of vitamin D by taking 1,600 IUs of vitamin D. • It takes a lot longer than you think to fix a vitamin D deficiency. • There are various things that prevent the absorption of vitamin D. Various things that can prevent the absorption of vitamin D: • Gut damage • Age • Skin pigment • Subcutaneous fat • Sun exposure • Metabolic issues • Season • Polymorphism • Chronic infection Vitamin D dosage: • You may want to consider taking 10,000 IUs of vitamin D per day for maintenance, but you may want to take more if there is an autoimmune issue involved. You would have to consume something like 100,000-200,000 IUs of vitamin D for months or years to create complications. The biggest complication of vitamin D toxicity could be hypercalcemia. Still, to help minimize the risk of hypercalcemia, you could drink 2.5 liters of water per day. You could also simply avoid consuming calcium in supplements as well as cheese. Per questa canzone bisogna pensare, e le parole sono la cosa più importante. “Ius Soli” parte dal presupposto che l’etica personale e sociale viene prima di ogni legge, perché è ciò che ci spinge ad occuparci degli altri e del pianeta: bisogna fare i conti con l’indifferenza e la violenza, con la paura del diverso, con leggi che respingono e confinano famiglie e comunità intere, e acconsentono a deturpare territori naturali. È una denuncia senza fronzoli contro noi stessi, contro la superficialità con la quale buttiamo bottiglie di plastica ed immondizia a terra o cambiamo vestiti e telefoni, contro lo stato che spesso si comporta più da azienda lobotomizzata che da difensore del diritto. Abbiamo “muri di carta e finestre di marmo” per non vedere la realtà e illuderci di essere al sicuro da un nemico che invece è dentro di noi. L’unica cosa che c’è da fare è ascoltare, osservare e ascoltare. Crearsi una coscienza. Il diritto ad esistere, il diritto della terra, l’altruismo, il soccorso verso popoli che vivono tragedie non dovrebbero essere leggi o concetti scritti, ma nella storia è sempre stato necessario guadagnarseli e lottare per essi. Ed è il momento di farlo per chi è già immobilizzato dal sistema. Dobbiamo credere che il mondo è uno, come diceva Bob Marley, che siamo un unico popolo, che la terra è la nostra casa e dobbiamo ricambiare la sua generosità quantomeno smettendo di sfruttarla e saccheggiarla.
